A private car from Ninh Binh to Ha Long takes about 3.5 hours and costs roughly 80 to 100 USD one way for a four-seater, with pickup at any address and a drop at your exact cruise port.
Split four ways, a 4-seater costs about 20 to 25 USD each, against 12 to 14 USD for a van seat. It's a little bit more expensive, so the premium buys a fixed departure time, door-to-port service and space for luggage. It is suitable for travel families with kids and elderly people, also group which cannot align their time with the normal car timeline, so of course, this is the most comfortable experience.
Ha Long Bay cruises board from several harbors: Tuan Chau on an island joined to the mainland by a long bridge, the newer Ha Long International Cruise Port in Bai Chay beside Sun World, and the smaller Hon Gai Pier near Bai Chay Bridge. Ask your cruise operator for the exact port before you book any transfer.

On the Halong Bay to Ninh Binh, vans collect passengers at the ports from about 11:30 AM to 12:15 PM and reach Ninh Binh around 4:00 to 4:30 PM. Time the pickup to your cruise's disembark time, and a direct van to Tam Coc usually beats a shuttle to Hanoi if Ninh Binh is your next stop.
Book the return in advance and give the operator your disembark time and port, because walk-up availability at the cruise ports can be limited. If your cruise includes a shuttle back to Hanoi, you can still skip it and take a direct van, since the Ninh Binh route does not pass through the capital. No direct train links Ninh Binh and Ha Long Bay, so rail means two legs through Hanoi. Ninh Binh to Hanoi takes about 2.5 hours on one of five daily departures for 2 to 10 USD, and second train toward Ha Long at about 7 hours with hard seats only.
That puts the full rail journey at 9 hours or more before you add transfers between stations and a taxi to the port, so it only works if you spend a night in the capital.
